HACC for under 65s

Home and Community Care Program for Younger People

The Home and Community Care Program (HACC) provides support for people under 65 with disabilities including the Aboriginal and Torres Strait Islander people.

Services are targeted to younger people with moderate, severe or profound disabilities and their unpaid carers.The aim is to maximise their independence, well-being and connections to the local community.

AccessHC offers a range of allied health and community services to those eligible for the HACC funding.

Allied Health government subsidised services open to HACC eligible adults include:

HACC services are available either free of charge or with a small co-contribution for eligible participants.

Dietetics

Provided by accredited practicing dietitians.

Services offered: Six monthly nutritional review, cooking education for disability housing staff, menu reviews (could be offered remotely for immediate start). These services can be delivered remotely.

Key audience are house coordinators:

  • requiring cooking education
  • interested in the provision of coordinated care to minimise the unnecessary loss of day care programs and cost of cancelling these programs.

We also provide coordinated care with other disciplines for oganisations, care co-ordinators or individuals benefiting from the provision of HACC services in their portfolio of care i.e. those who do not have diet as part of their NDIS plan.

  • Doncaster East
  • Richmond
  • Hawthorn.

Outreach services provided in the Cities of Boroondara, Mannigham and Yarra.

Podiatry

Services offered:

    1. Annual foot assessment
    2. Regular review consultations
    3. Assessment and advice regarding appropriate footwear
    4. Coordinated care with disability housing and other disciplines.

Key audience:

  1.  Organisations seeking to add Podiatry to their service portfolio e.g. DAS Houses.
    •  Services could be offered on outreach basis with a podiatrist/s having a designated day/s to provide service on location at a DAS house
    • A day activity program offering clients access to a high quality, consistent service could still be offered if the client transitions to NDIS – having podiatry included in their NDIS plan
  2.  Clients who qualify for NDIS but do not have podiatry in their current plan and cannot make low/out of pocket HACC payments. Service could be provided under Medicare Team Care Arrangements until included in NDIS plan.  Podiatrist can supply report to evidence need for ongoing podiatry to be included at scheduled NDIS plan review.

      • Balwyn North (currently closed but will reopen when Covid-19 restrictions allow it)
      • Richmond
      • Hawthorn
      • Ashburton.

Outreach services provided in the Cities of Boroondara, Mannigham and Yarra.

Physiotherapy

Services offered:

  1. Individual clinic appointments – with an acute health condition
  2. Home visit appointments
  3. Group programs.

Key audience:

  1. Clients who have a health related need that is not covered by NDIS e.g. acute injury
  2. People under 65 years of age who are not eligible for NDIS with reduced mobility, function or pain.

We have a variety of programs, with a mixture of short term or ongoing. They are currently being run through telehealth so client needs to have a computer with camera, iPad or smart phone to be able to participate.

  • Doncaster East
  • Richmond
  • Hawthorn.

Outreach services provided in the Cities of Boroondara, Mannigham and Yarra.

Occupational Therapy

Services offered:

  1.  Home visit appointments
  2. Telehealth phone or video consults
  3. Wheelchair clinic

Key audience:

  1. Supporting clients with chronic pain or fatigue who require functional assessment and professional reports to support NDIS applications
  2. People under 65 years of age who are not eligible for NDIS with reduced mobility, function or pain, or physical and mental health illness co-morbidities, e.g. chronic fatigue, fibromyalgia, depression, anxiety
  3. Palliative management for clients not on NDIS (palliative services).

We currently offer telehealth or face-to-face appointments. If appointment run via telehealth client needs to have a computer with camera, iPad or smart phone to be able to participate. Wheel Chair Assessment Clinic at Balwyn North site currently closed but will reopen when Covid19 restrictions allow it.

  • Richmond
  • Hawthorn.

Outreach services provided in the Cities of Boroondara, Mannigham and Yarra.

Speech Therapy

Services offered:

  1. Clinic or home based appointments
  2. Telehealth phone or video consults
  3. All standard assessment and intervention follow up available.

Key audience:

  1.  Clients with sudden onset and rapid progression of rare diseases. Case example 55 year old with Creutzfeldt-Jakob disease where access to NDIS may not be possible in the timeframe from diagnosis to time of death
  2. Clients who do not qualify for NDIS:
    • with atypical presentations that do not qualify for NDIS (case example – some voice referrals or unexplained dysphagia) or require urgent engagement while transitioning to NDIS
    • typically based on consideration of ‘permanent’ disability where status fluctuates. Case example was 35 year old with Chronic Fatigue disorder
  3.  Clients who are unable to seek help with the process of transferring to NDIS or declined to transition. Case example client with Glossectomy – removal of tongue.
