We are passionate about helping you transition to the NDIS. We will assist you to achieve your goals and reach your potential.

The National Disability Insurance Scheme (NDIS) is about giving greater choice and control to you, your family and carers. You will have greater choice over how your supports are delivered. Supports may include allied health and therapy services, equipment, and getting involved in the community.

NDIS eligibility

You can access the NDIS if you are:

  • A parent of a child with a developmental delay
  • A person with a disability (including a psychosocial or mental health disability), or developmental delay
  • Under 65 and a permanent resident of Australia

How Access Health and Community can help

Our doctors and clinicians can help you to access the NDIS by:

  • Assessing your current needs
  • Providing supporting documentation about your disability or developmental delay and how this impacts on your ability to take part in everyday activities
  • Helping with the NDIS Access Request form
  • Advocating on your behalf regarding your future needs
