This service aims to improve client wellbeing and maintain the client’s independence and capacity to live safely at home and participate in community activities and services.

Supported connection
Supported connection provides:
- community engagement workers who build trust and capacity of Home and Community Care Program for Younger People (HACC PYP) eligible clients so that they can access services to meet their health and wellbeing needs
- outreaching through a Community Engagement Worker to build capacity of clients to engage with services, such as warm referral and service introductions.
Eligibility
The program is currently available across Local Government Areas (LGAs) of Boroondara and Manningham for people who:
- are aged under 65 or Aboriginal people aged under 50 who need some assistance or support with activities of daily living to remain living independently in their home/community
- have psychosocial needs, chronic illness or short-term health and wellbeing needs and require HACC-PYP services intermittently
- are carers.
We support HACC PYP eligible clients to overcome barriers to using HACC PYP services.
Access to Community
Access to Community is a social prescription program offered by Access Health and Community that links people with a volunteer community connector to support social connection and belonging.
This work takes place through collaboration with community groups and meets people at places such as: libraries, cultural groups, emergency food relief, and youth services.
Service Connectors Outreach
This outreach service is for people aged over 65.
Service Connectors help people to register with My Aged Care.
Service Connectors can also help to link people with community activities. This can be the My Aged Care entry point for people over 65 who don’t have anyone to help them register and need assistance.
