Pathway to Good Health

Pathway to Good Health is a program helping to provide children and young people who are entering out-of-home care with the best possible health care.

The program provides early access to comprehensive health care and developmental assessments for the children aged 0-17 years old.

Multidisciplinary team

Health Navigators provide assessments and develop an individualised and targeted health care plans for the children.

The program is delivered by a specialist multidisciplinary team including a:

  • Paediatrician
  • Speech Pathologist
  • Occupational Therapist
  • Child Psychologist.

The team work together to deliver timely, collaborative, trauma-informed and child-centred care.

Who can use this service?

Children and young people are referred to Pathway to Good Health by a Child Protection Case Manager.

How much does it cost?

This is a free service.

Outer East

The Outer East Pathway to Good Health clinic is run in Ringwood.

Pathway to Good Health in the Outer East is delivered through a partnership between: Each, Eastern Health and Access Health and Community.

Inner east

The inner east Pathway to Good Health clinic is run in Box Hill.

Pathway to Good Health in the Inner East is delivered through a partnership between LaTrobe Community Health, healthability, Eastern Health and Access Health and Community.
