Community Health First unites all registered community health services with one shared goal – improving the health, wellbeing and quality of life for all Victorians.
As part of this important work, we would like to hear from you about your experience of receiving health and wellbeing services over the past year from Access Health and Community.
Your answers are confidential and will help us understand the difference community health is making in helping to keep you healthy and well.
