Want to give us feedback?

Your feedback tells us what we are doing well and where we need to improve. It will not disadvantage you or the care we provide.

How to give us feedback

To give your feedback:

  • speak directly to our staff
  • fill in the feedback form online
  • call us on 9810 3000 and ask to speak to the manager of the service you attended
  • email info@accesshc.org.au
  • fill in the printable Feedback form (also available at reception) and put it in the feedback box at reception, or mail it to: 378 Burwood Road, Hawthorn, VIC, 3122

For unresolved complaints: 

An independent disability advocate can help you make sure your voice is heard when you make a complaint.

They can speak, act or write on behalf of a person with disability. They are independent of the NDIA, NDIS Commission and all NDIS providers.

To find an advocate – please use the Disability Advocacy Finder (compatible browsers include Microsoft Edge or Google Chrome, not available using Internet Explorer).
