Access Health and Community Outreach

AccessHC connects individuals and families with healthcare, services and supports, particularly those facing barriers to access.

What do we do?

The Access and Outreach team can: 

  • meet people in community settings that suit the person 
  • make referrals 
  • help make and attend appointments – for example, planning travel, making introductions and settling in 
  • help to fill out forms 
  • link people with social activities. 

Who can we help? 

Services are available to people at different stages of life who find it difficult to access or navigate service systems on their own. Some community members may experience barriers to accessing support and may:

  • be reluctant to engage with health systems or government systems 
  • find it difficult to fill out forms or use online forms 
  • have limited support from family, friends or carers
  • have difficulties with communication 
  • feel unsafe or vulnerable. 

If you or someone you know needs support accessing services, our team can help connect you with the right information, supports and community services.

We have a range of programs that can assist with connection.

Outreach  program

Our Outreach team works with all ages. This service is for people experiencing a combination of hurdles.  

Our Outreach team can meet people in the community and help to find and link with the right support. This friendly team includes community health nursing and social work.

Access to Community (A2C) program

Access to Community social prescription program links people aged under 65 with a volunteer Community Connector. The aim of this program is to connect people with existing community activities, with the support of a volunteer. Volunteers are also great to practice having conversations and warming up for the next steps of joining social situations.

Service Connectors – Care Finders Program

This outreach service is for people aged over 65. Service Connectors help people to register with My Aged Care. Service Connectors can also help to link people with community activities. This can be the My Aged Care entry point for people over 65 who don’t have anyone to help them register and need assistance navigate the system.
